Notification of a Positive Covid-19 Case

Any member of PDMHA who tests positive for Covid-19 must notify the association by completing and submitting the Notification of a Positive Covid-19 Case Form.  Information submitted through the form will be kept confidential; PDMHA will contact SouthWestern Public Health and follow their directives regarding the case.

Ontario Vaccination Policy

As of Sept. 22 2021, any individual aged 12 and older must be fully vaccinated to enter Plattsville arena (and many other public facilities and businesses).  The only exception is for youth 12 to 17 years of age, who are actively participating in sporting activities - while the players themselves do not need to be fully vaccinated (until Oct. 31; see OMHA policy below), any accompanying parents, siblings, etc, must have a status of being fully vaccinated or have a doctor's note to verify a medical exemption.

OMHA Vaccination Policy

OMHA has decided to proceed with its original policy and is mandating that all players aged 12 years and older, coaches, officials, and volunteers are fully vaccinated by Oct. 31 2021. If not fully vaccinated, the individual will be suspended from any further participation in minor hockey activities, as of Nov. 1, until they become fully vaccinated.

Any affected individual may apply for an accommodation exemption to this mandate, with medical documentation, directly with OMHA; if approved, the accommodation exemption can then be provided to PDMHA.

Providing Proof of Vaccination

"Fully vaccinated" means that it has been at least 14 days since a second dose of an approved vaccine (Pfizer, Moderna, AstraZeneca), or since a single dose of the Janssen (Johnson & Johnson) vaccine. 

To obtain your vaccine certificate QR Code:
  • go to
  • click on "Get your enhanced vaccine certificate" and follow the instructions
  • save your QR code to your mobile device, or print a copy
  • show your QR code at the door, along with a piece of photo identification

To provide proof of vaccination:
  • go to
  • scroll down, agree to the Terms of Use and click on Continue
  • enter the required information for the individual (requires Health Card info, date of birth, and postal code), then follow the prompts to obtain an electronic copy of vaccination receipts
  • show your receipts, along with photo ID, to Tina Balfour (at the check-in table during scheduled ice times)
  • the individual(s) will then be added to a master list;  receipts/ID will not need to be presented thereafter
