Coaching Application, 2025-26 Season
Please complete and submit the following information, to indicate your interest in a coaching position for the 2025-26 season. Note that actual team divisions/levels offered will be dependent on sufficient registrations. Certification

* I acknowledge that if selected for a coaching role, or a supporting bench staff position, I will provide PDMHA with a Criminal Record background check (including Vulnerable Sector Check) that has been completed since August 2023.  At minimum, proof of application for such a background check will be provided no later than Sept. 1 2025.

Declaration

Declaration: I agree that if selected, I will upgrade any coaching skills as requested by PDMHA, attend all Coaches' meetings, abide by the PDMHA Constitution and OMHA Code of Conduct, comply with all rules and regulations of PDMHA and the controlling bodies, and undergo a Criminal Record / Vulnerable Sector Background Check (if a new team official with PDMHA or if such a Check has not completed within the past 3 years). I also agree to, wherever possible, promote the playing of hockey at its highest level. I will accept full responsibility for my actions while acting as a team official of the PDMHA.
