Pre-Registration for 2021-22
Please complete the following to pre-register your player(s) for the 2021-22 hockey season. No monies will be collected at this time, and pre-registrations may be cancelled at any time, up until Mon. Sept. 6th, with no penalties.

If any boxes are checked for the above question, it is only an indication of the player's possible interest in trying out for a AAA/AA/A level team. It is the player's/family's responsibility to contact PDMHA with any requests for Permission to Skate forms and/or Non-Resident Player passports, at least 48 hours prior to any scheduled tryout sessions at such AAA/AA/A centres.
