Pre-Season Skills Mini Camp
Please complete the following, to register for the Pre-Season Skills Mini-Camp. Cost is $40/player and includes four hours of ice time over two sessions. Players MUST be registered with PDMHA for the 2018-19 season. Participant One

Please indicate which age group the player will be attending. Peewees are welcome to attend either age group's sessions. Group 1: Novice, Atom, Peewee (birth years 2011-2006), Friday Sept 14 5:00-6:50pm and Saturday Sept 15 9:00-10:50am. Group 2: Peewee/Bantam/Midget (birth years 2007-2001), Friday Sept 14 7:00-8:50pm and Saturday Sept 15 11:00am-12:50pm

The undersigned is responsible for the conduct of the player while participating in this event. It is understood that the undersigned person is of legal age or a legal guardian, and shall not hold the program/event or its organizers, officials or facility used, liable in the event of injury or loss in any manner whatsoever.

I also certify that my child is in good health with no chronic illness or abnormal tendencies. The player listed above is registered under the care of the undersigned who assumes all risks through the enrollment in the program/event which consists of physical interaction capable of injury.

The player must be registered with Plattsville & District Minor Hockey Association, and wear full hockey equipment during the on-ice sessions.
