Please complete and submit the following form in order to attend PDMHA evaluations/tryouts. Fee is $25/player; at least 2 sessions guaranteed, before any cuts made. Players coming from A/AA centres must have submitted a Non-Resident Player passport (NRP; U13 or U15) or an Authorization to Move form (ATM; U15).
