PDMHA Golf Tournament Registration
Complete and submit this form to register up to four individuals for the PDMHA Golf Tournament, to take place on Sunday June 11 at Dundee Country Club. Participants must be 12 years of age of older (note each player under the age of 16 must be accompanied by an adult).

Golf Membership

If any player being registered here has a multi-course membership with Golf North for Dundee, Foxwood, Rebel Creek, Conestoga, or Grey Silo clubs, please indicate their names below - cost for our tournament is $50 for such players; membership will be confirmed by Golf North prior to the tournament.
