Please complete and submit the following form in order to attend PDMHA tryouts. Payment must be received at the door before players will be allowed to participate. Players coming from A/AA centres must bring a Non-Resident Player passport with them; Midget-aged players coming from other centres must bring their Authorization to Move form with them.
